How to Apply for Admission

If you or someone you know is in need of substance use treatment, here is the process to find out if Wells House is the best placement for you.  The process begins with a Prescreen Application filled out completely and legibly by the person seeking treatment.  Here is how to obtain and submit an application to the Wells House program:

• Call to request a Prescreen Application Form (we can mail or email it to you)
  • 301-662-7003 for Frederick
  • 301-739-7748 for Hagerstown
• Or, go to the website https://www.wellshouse.org, to the Admission tab, to How to Apply for Admission and scroll down to Wells House Prescreen Application 2021. You can fill it out and submit it online from a laptop or tablet.   You will not be able to submit this application via a cell phone.  Or, go to the application on the website, download it, fill it out and email it to info@wellshouse.org
